Submit 24 CEUs per two-year cycle via:

CEU Submission Form for IN-PERSON EVENTS at Physical Locations

• Please use this CEU form (download here) for attendance of live in-person events at physical locations.
• Complete the form and have it signed by the Host or Speaker, and upload the completed form to the “Files” section in your Instructor Portal.

CEU Online Submission Form for WEBINARS (“secret words”)

• Please complete this online form to submit the secret words given to you during a webinar.
